If you choose to upload any insurance or financial verification data, this information is collected and used as part of the intake process for possible sliding fee availability.
While the Agency takes data security and confidentially very seriously, no transmission of data over the internet or any wireless network can be guaranteed to be 100% secure. The Agency use a number of electronic and procedural security measure to protect the confidentially of any information submitted through this site. Additionally, all information submitted through this site employs encrypted transmission over the internet.
The Agency will only share any information through the use of this site with third parties (i) when the person submitting the information authorizes the Agency to share it; (ii) as set forth in the Notice of Privacy Practices (iii) when sharing the information is necessary for the operation of the site, or (iv) when we are required to by law. The Agency will never sell the information collected.
